Established Patients

APPOINTMENTS

• If you'd like to schedule an appointment, please to do so here.  This is for established patients only.

• If you have not seen me since November 2023, Athena portal will not allow you to send me a message, so please send a text or message through Spruce instead. You can still book an appointment, as you have an Athena account.

REFILL REQUESTS

• Please request all refills through the portal.  

• Please make sure your pharmacy is up to date. 

• If you are on a controlled substance, I must see you every 3 months.  All other refills require visits every 6 months. 

• All patients must be seen in person at least once per year.

PAYMENT

• There is no billing through Athena.  All billing will be done through Hint Health.  

• Payment is due at the time of service or prepaid in the form of membership. 

• If you prefer labs to be billed through me ("cash based labs"), you must tell me prior to having your labs drawn.  

• Payment for supplements or functional testing is due upon receipt. 

LABS

• I have accounts at both Quest and LabCorp.  Click the appropriate link to book your lab appointment. 

• I have cash based pricing at Quest only.  This means, if your insurance cost for labs is unsatisfactory for you, you can pay me directly for the labs and I will submit the fee to Quest. You must let me know at the time of your lab draw that you would like to switch the billing.

• I am not responsible for your lab bill or what is billed to you after insurance reimbursement.  The price your insurance contracts with the lab is between your insurance and the lab.  This includes for any Pap test, vaginal swabs, cultures, or biopsies taken in the office. 

• If you would like a quote for cash based labs that we have discussed, please let me know and I am happy to create this for you. 

FEES

• Please be advised that Camel City Women's Wellness, is a concierge direct primary care office. Those who are on membership plans, incur no additional fees for visits, annual physicals, or communication with their provider during or after hours.  

• All memberships purchased prior to February 23, 2024 are granted legacy pricing.  As of February 23, 2024, membership pricing is below.

• Memberships are 3 month commitment minumum. 

• If you want to change your payment structure or payment source, you can do so directly from your Hint account.  If you need help, please email admin@camelcitywomenswellness.com, call or text 336-585-7312.
